Nasal Surgery and Consultation

During a rhinoplasty consultation, Dr. Perenack will meet with you and discuss your goals and review your desired alterations. Recommendations will be made based on your examination and your anticipated results. He will provide you with the personal attention needed before surgery so that you feel comfortable and educated about your decision. The procedure can be performed under local anesthesia, twilight sedation or general anesthesia.

Dr. Perenack approaches this outpatient surgery with the goal of balancing the face with proportionate facial structures and maintain or improve your ability to breath. Rhinoplasty reflects each patient’s unique personality while enhancing the facial silhouette at every angle. The nose job is in essence surgical sculpting of the nose and is accomplished by careful contouring of the bone and cartilage giving the nose its shape. At the same time deformities that cause breathing problems and some sinus disorders can be corrected.

  • What changes can be made?

    Common cosmetic goals include smoothing a nasal hump, refining a bulbous or drooping tip, narrowing the bridge or nostrils, and correcting asymmetry. Functional goals often involve correcting a deviated septum to improve breathing.

  • Am I a good candidate?

    Good candidates are typically in good overall health, do not smoke, and have realistic expectations for the outcome. For younger patients, the nose and face should be fully grown, generally around ages 15-16 for girls and 17-18 for boys.

  • What is the difference between open and closed rhinoplasty?

    Closed Rhinoplasty: Incisions are made inside the nostrils, leaving no visible external scarring. This is typically used for less extensive changes.


    Open Rhinoplasty: An additional small incision is made across the columella (the strip of tissue between the nostrils), allowing the surgeon to lift the skin and fully view the internal structures. This is used for more complex or extensive procedures and leaves a small, usually inconspicuous scar.

  • How long does the surgery take?

    The procedure typically takes between 1 to 3 hours, depending on the complexity of the case.

  • Is it painful, and what kind of anesthesia is used?

    The surgery is usually performed under general anesthesia (the patient is asleep) or heavy sedation, so there is no pain during the procedure. Post-operative discomfort is usually mild to moderate and managed with pain medication.

  • How long is the recovery?

    Immediate Recovery (1 week): A nasal splint or cast is usually worn for about a week. Swelling and bruising around the eyes and nose are common and peak in the first few days.


    Returning to Activities (1-2 weeks): Most patients return to work or school after the splint is removed (around 7-10 days), though swelling and bruising may still be noticeable.


    Vigorous Exercise (6 weeks): Strenuous activity is typically avoided for about six weeks to prevent injury and prolonged swelling.


    Final Results (Up to 1 year): While major swelling subsides within a few weeks to months, the final, subtle refinements of the nose's shape can take up to a full year (or sometimes more) as all residual swelling resolves.

  • Will I have visible scars?

    With closed rhinoplasty, there are no external scars. With open rhinoplasty, a small, fine scar is left on the underside of the nose (columella) but is generally well-hidden and fades over time.
