Graduating in 2005 from a New Orleans-based medical institution placed Dr. Picarella at a unique historical and clinical inflection point.     Internship in General Surgery

    Dr. Picarella commenced his post-graduate medical training with a rigorous internship in General Surgery at the University of Mississippi Medical Center. The surgical internship year—often referred to historically as the PGY-1 (Post-Graduate Year 1) year—is universally recognized within the medical community as one of the most physically and psychologically demanding periods in a physician's training. During this year, the newly minted Dr. Picarella was tasked with managing the minute-to-minute care of critically ill surgical patients on the hospital wards, assisting attending surgeons in major trauma and elective cases, and developing the fundamental kinesthetic and cognitive skills required in the operating theater. 


    Residency in General Surgery and Chief Administrative Role

    Following the successful completion of his intern year, Dr. Picarella was selected to continue his highly competitive residency in General Surgery at UMMC. General surgery residency is a multi-year, grueling crucible designed to train physicians to independently and masterfully manage diseases of the alimentary tract, abdomen, breast, endocrine system, and critical trauma. Over the course of this multi-year program, Dr. Picarella performed thousands of surgical interventions, ranging from emergent laparotomies for blunt abdominal trauma to complex resections for gastrointestinal malignancies.During his tenure in the general surgery program at UMMC, Dr. Picarella distinguished himself not only through his superior surgical acumen and manual dexterity but also through his exceptional capacity for leadership, peer mentorship, and institutional stewardship. In recognition of these qualities, he was appointed to the esteemed position of Chief Administrative Resident.


    In this capacity, Dr. Picarella operated as the primary liaison between the attending surgical faculty and the resident body. His responsibilities were vast and complex, including the management of the intricate surgical schedules for dozens of junior residents, ensuring strict institutional compliance with the Accreditation Council for Graduate Medical Education (ACGME) duty-hour standards, mediating intra-departmental conflicts, and actively overseeing the pedagogical and technical development of junior surgeons.

  • Advanced Subspecialization: Fellowship in Plastic and Reconstructive Surgery

    Upon the successful and highly commended completion of his General Surgery residency, Dr. Picarella pursued the pinnacle of surgical sub-specialization by securing a coveted fellowship position in Plastic and Reconstructive Surgery, electing to remain at the University of Mississippi Medical Center to complete this rigorous training. He successfully completed this exhaustive fellowship training program in 2013.

His advanced training encompassed the complex reconstruction of severe traumatic injuries—often requiring close collaboration with orthopedic and neurosurgical trauma teams to provide soft-tissue coverage over exposed bone, hardware, or neural structures. He trained extensively in head and neck cancer reconstruction, utilizing autologous tissue flaps to rebuild jaws, tongues, and facial structures following radical oncological resections. Furthermore, his fellowship heavily emphasized breast reconstruction following mastectomy, mastering both implant-based techniques and highly complex autologous tissue transfers (such as TRAM or DIEP flaps), which require the precise anastomosis of blood vessels smaller than a millimeter in diameter under an operating microscope.Simultaneously, Dr. Picarella refined the meticulous, nuanced techniques of cosmetic aesthetic surgery, mastering the complex vectors of facial rejuvenation, the architectural principles of rhinoplasty, and the volumetric dynamics of body contouring.
